Care Plan Philosophy

At Emmaus House the emphasis is placed on high quality individual person centred care with attention to individual service users needs, preserving their dignity and identity at all times.

To ensure that we deliver the highest possible service provision each service user is treated as a unique individual with a plan of care that reflects their specific needs.

On admission an accurate and comprehensive assessment will be carried out and will be followed up with regular reassessments as often as the service user's status demands.  Your care plan will be reviewed at least once a month to reflect any changes in your personal needs and will comply with the Latest Standard of Quality & Safety.

You will be actively encouraged to be involved in the planning of your care at all times and if you wish you may also involve members of your family, friends, advocates and other agencies in the planning and delivery of your care.

In our endeavour to provide you with optimal quality of life, comfort and dignity your care plan will be unique and take account of any special conditions such as e.g. Alzheimer's disease, Dementia, Nutritional problems to mention a few.

Finally, it is important to remember that the purpose of the care plan is to guide all who are involved in your care to provide the best possible service therefore, your plan should contain all the information needed to care for you.
